Sure Match DNA Testing Policy
Thank you for choosing Sure Match DNA for your DNA testing needs. In order to ensure a smooth and accurate testing process, please read and follow the guidelines below.
1. Identification Requirements:

Valid ID: All testers must provide a valid government-issued ID (e.g., driver’s license or passport). The name on the ID must match the name of the person taking the test.

Minor Children: If the test includes a minor child, a valid birth certificate is required.

Social Security Number: In some cases, a social security number may be required for verification purposes.
2. Test Preparation:

No Drugs, Alcohol, Nicotine, or Vaping: Testers must not consume drugs, alcohol, nicotine, or vaping products prior to the test.

Eating and Drinking: Do not eat, drink, chew gum, or brush your teeth for at least 30 minutes before collecting the sample.
3. Attendance:

Testers Must Be Present: All test participants must be present at the appointed time. No exceptions will be made.
4. Chain of Custody:

Photographs: Photographs of the testers may be taken to ensure chain of custody, especially for legal (court-ordered) tests. These photographs will be securely stored as part of the testing records.
5. Refunds:

No Refunds: Please note that once your test is scheduled and the sample collection process has begun, there will be no refunds under any circumstances.
6. Test Results and Liability:

Test Results: Sure Match DNA is not responsible for any undesired or inconclusive results that may arise from the test.

Spoiled Tests: Sure Match DNA is not liable for any tests that are spoiled due to improper sample collection or failure to follow pre-test instructions.
